Welcome to Allure Cosmetic Laser Center. Thank you for selecting our clinic for your skin care needs – we appreciate your confidence in us. The following information is for you to review.

Appointments

To schedule appointments, please call our office at 702.304.0336 during office hours. Once your appointment is scheduled, you will be given access to our online patient portal (link is below) that will allow you to enter or update your demographic, medical and financial information. Please be sure to complete this process before you arrive at the office for your appointment.

If you need to reschedule or cancel your appointment please note that we require a 24-hour notification. If you are unable to keep your appointment and do not call to cancel the appointment, you will be considered a “no show”. It is our office policy to dismiss a patient from the practice when they have failed to arrive for 2 appointments.

Allure Cosmetic Laser Center seeks to provide timely service and consideration to all of our patients. There will be occasions when emergencies or procedures that run longer than anticipated will create longer than usual wait times. If this should happen, we will make our best effort to keep you informed and to get back on schedule. In addition, if you should arrive more than 15 minutes late you may be asked to reschedule.

Cosmetic Appointments

Allure Cosmetic Laser Center strives to provide the latest innovative procedures and quality service. Your first appointment will begin with a meeting with one of our cosmetic consultants where she will gather your information and review with you your goals.

Once the cosmetic consultant has gathered the information needed, you will meet and spend time with Dr. Capobianco. Dr. Capobianco will examine you and review with you your options to determine which procedures would be best for you. Together, you will select a treatment plan to achieve your desired outcome. She will answer any questions about your treatment and ensure that you are completely comfortable.

Once you are comfortable with the treatment plan that you and Dr Capobianco have decided upon, the cosmetic consultant will give you a quote and you will schedule your treatment. Cosmetic procedures are normally scheduled for future dates, but time permitting, you may be able to receive treatment on the same day as the consultation.

Cosmetic procedures are never covered by your health insurance plan; therefore, you will be required to pay for the service in full or leave a non-refundable deposit PRIOR to the scheduled treatment date. Our cosmetic consultant will review with you all of your payment options and deposit requirements at the time of your consultation. A cancellation of a cosmetic treatment will forfeit your deposit and a new deposit will be required before the appointment will be rescheduled.

Aesthetician Appointments

Our licensed aesthetician can design a treatment program that’s right for your skin. He will review your concerns, your current regimen, evaluate your skin and skin type, and then offer a suggested plan of care. You are encouraged to bring in the products you are currently using for evaluation.

Appointments are scheduled in 30 or 60 minute intervals. If it is necessary to cancel an appointment, we require a 24-hour notification; otherwise, you will be responsible for the full fee for the service.

Emergencies

If you should experience a skin emergency after hours please call our office and follow the prompts to leave a message for the physician on call or call 911.

Prescription Refills

If you should require a prescription refill, please call our office and leave a message in our prescription refill voice mailbox. Please have the name of the prescription and a pharmacy phone number ready. Allow for a turn around time of 24 hours. If you need a new prescription or have questions about a medication you are presently taking, please leave a detailed message and a phone number where we can contact you on the nurses voicemail. The nursing staff at Allure Cosmetic Laser Center works hard to ensure that all calls are returned in a timely manner.

Biopsy Results

If you should have a biopsy done in our clinic, please note that within two weeks of your visit you will receive an e-mail, a phone call or a letter in the regular mail explaining the results of your test. If you have not received your results within a 3-week period, please notify our office immediatey.

Financial Policy

Allure Cosmetic Laser Center has contracts with the following insurance companies: MEDCOST, BCBS, MEDICARE, CIGNA, UNITED HEALTHCARE

  • It is the patient’s responsibility to confirm that Allure Cosmetic Laser Center has a contract with your specific insurance plan. As a courtesy to our patients we will file claims to the above insurance companies. You will be responsible for any amount unpaid by your insurance at 30 days. Additionally, you will be asked to pay for all deductible, copay and co-insurance amounts at the time of service.
  • Allure Cosmetic Laser Center may perform, at the patient’s request, a service that is considered not medically necessary (cosmetic service) by the patient’s insurance plan. This falls under the title of “Non-covered Services”. If the patient should receive such a service, the patient will be held responsible for payment to Allure Cosmetic Laser Center of that procedure at the time the service is performed.
    • Examples of Non-Covered Services include but are not limited to: laser procedures, skin tag or benign mole removal, and cryosurgery of benign spots.
    • All pre scheduled cosmetic services require full payment or a deposit before the service will be entered in to our appointment calendar.
  • Allure Cosmetic Laser Center accepts Visa, MasterCard, Discover, debit cards or cash as methods of payment. NO PERSONAL CHECKS ARE ACCEPTED. The only exception is for planned cosmetic surgical procedures where patients may choose to submit a personal check for payment 72 hours in advance of surgery.
